The following forms are needed to sign up for SANTOS FC.

Santos Application

USA Youth Membership Form.  (Three signatures are required)

Medical/Dental and Waiver of Liability Form (This form must be notarized in the presence of a Notary Public)

Parent's Code of Conduct Contract

Picture - must be 1" x 1", face forward and in color - can't cut off hair, ears or chin.  (new picture every year)

Uniform Order Form  BOYS     GIRLS

Proof of Birth - only if you are new to Santos - Copy of Birth Certificate or copy of Passport

Checks:  Fees shall be made payable to SANTOS FC; Uniforms and Spirit wear to TVSA (Tri Village Soccer Association)
